Youth Volunteer Application/Affidavit

The Rotary Youth Programs appreciate your interest in serving as a volunteer, and we are confident that this will be a truly rewarding experience for you.

Rotary International, and District 5050 require that we obtain the information on this form and conduct background checks on all potential volunteers. We assure you that the information that we receive will be treated confidentially, and we thank you for your assistance and understanding.
You will need the following items for your Application: A photo of yourself, and three personal references (name, phone and email). Once your application is submitted you will recieve the training module(s) via email and other instructions

Note that each Youth Program may have slightly different requirements.
